Steps of the Assistive Technology Process:
If you need to begin using a mobility device (wheelchair, scooter, power wheelchair), replace your current device, or make a change to your current device, we’ll follow the outlined steps below:
- Your physician will need to write a prescription for “PT/OT Evaluate and Treat for wheelchair/assistive technology assessment”. Please have the office fax it to Life Wheels On at 719-398-0794.
- You will choose a supplier if you have not done so already. This is the company who will order the wheelchair. A representative from the company, commonly referred to as the ATP, will work with you and your LWO therapist to determine your equipment needs.
- Your LWO therapist will work with you, your supplier and your physician to help you obtain the correct equipment and documentation for your insurance company.
- Your LWO therapist will see you after the equipment is delivered to make sure it is meeting your mobility goals and perform training on the device as needed.
