Steps of the Assistive Technology Process:

If you need to begin using a mobility device (wheelchair, scooter, power wheelchair), replace your current device, or make a change to your current device, we’ll follow the outlined steps below:

  1. Your physician will need to write a prescription for “PT Evaluate and Treat for Assistive Technology Needs”. Please have the office fax it to Life Wheels On at 719-398-0794.
  2. You will choose a supplier if you have not done so already. This is the company who will order the wheelchair.  A representative from the company, commonly referred to as the ATP, will work with you and your therapist to determine your equipment needs.
  3. The PT from Life Wheels On will work with you, your supplier and your physician to help you obtain the correct equipment and documentation for your insurance company.
  4. The PT will see you after the equipment is delivered to make sure it is meeting your mobility goals and perform training on the device as needed.

The average total time for the steps above is 3-6 months.
